As an on and off heroin user of many years i can tell you what worked for me best the last time. I took one 8mg the first day. 4mg the second. 2mg the third. 1mg the forth. .5mg the fifth and then discontinued using. Suboxone is best as a quick tapered process. Be warned you feel some slight discomfort but thats the price of using opiates. I feel as though long term use of suboxone doesnt help me at all and have found the withdrawals from prolonged use worse than the initial opiate withdrawals. Your use of opiates wasnt on the high end so this method should do you find. I am in no way a doctor or medical professional so always consult your doctor first on serious issues.
