I'll try to give it a shot. In the meantime, here is a past (and future) blog post, where I give my thoughts about how 'the steps' compare to medication-based recovery:

Keep the questions coming....

Hey Dr. J.,

I've always wondered why the elimination half life of Suboxone (buprenorphine) varies so widely. The numbers I've seen say the elimination half life is anywhere from 20 - 73 hours.

Do most medications have half lives that vary so widely?

I agree with you in large part that clients adequately maintained on buprenorphine should not be required to attend mandatory counseling; there are even arguments that when maintenance patients who do not utilize counseling and similar populations either required or choose to attend counseling along with maintenance seem to have very similar rates of success. Do you think that short-term and occasionally longer-term professional counseling plays a role within the overall treatment plan of certain maintenance patients and if so, what would be some issues that would make you feel as though counseling might be beneficial for a particular patient? What types of counseling have you noticed to have noteworthy benefit, if any, or what have your counseling-referred clients expressed as seemingly being beneficial to them within the dynamic they have (hopefully) formed with their clinician? Thanks, Doc!
