Hi:

Been lurking, first post in a while :)

Short story: Had five knee and shoulder surgeries and ended up on way way too high of a dose of Oxycodone (~175mg per day). And this is after tapering for 9 months -- I was on as much as 500mg per day. Am working with a Dr at a pain management center. He want me to switch to Subutex or Suboxone. His protocol is 2.5- 3 day hospital stay, with 48 to 72 hours of waiting with no meds before starting the Subs.

Met new Dr, who is a psychiatrist specializing in addiction. This new guy has been practicing for 25+ years. He has new protocol for induction from Oxycodone/Oxycontin. ~ 12-16 hour wait, get to 6 on the COWs scale, add 2-3 more hours, start at 20% dosage of Suboxone., wait several hours, get back to 6-10 on COW's, add additional dose of Suboxone, repeat till stable (i.e. after last dose of Suboxone, no increase of withdrawal symptoms. I have a rock solid referral to this new Dr from someone I trust.

I know this is different. Anyone have any experience with this protocol? New Dr says it works well, he's been using this protocol for years now. Much easier, still effective. He also tapers from Suboxone in 2-4 months, if possible, but not required.

Hi Mr_c,

Welcome to the forum. It sounds about right. Doctors used to make Oxy patients wait for too long in the past before induction. Sounds like this new Dr. is on the ball.

Did you ask your pain doctor how your pain would be managed while on Buprenorphine? Some do well with small doses throughout the day. Some others say it doesn't work as well as a full agonist like Oxycodone. Personally, I have no opinion about it.

The induction sounds good. Just make sure they can still manage your pain.
