I'm sorry to hear you're in such chronic pain. I know from personal experience that pain like that invades every part of your life. Do you only dose once daily? Sub's pain properties only last on average 6 hours, so you might try to dose 2-3 times per day instead of once daily. Of course it's possible that even doing this won't help your pain enough.

Have you discussed using methadone instead of bupe? Methadone is said to be much better at treating pain. It's the only thing I can think of instead of going back on pain pills. Just a thought.

I wish you the very best.

Have you gone to a "GOOD" pain management clinic or seen a "GOOD" pain management specialist? There are often more things they can try other than opiate drugs. For example, for compression fractures (I assume that is what the flat tire thing is) there is some great "bone cement" techniques that many people have had success with. They go in under low level radiograph (called flurral) and inject "bone cement" to "re-inflate" the disc which then takes pressure off of the nerves and relieves the pain. This is just one potential "fix" for you.

If you have not explored some of the growing number of non-medication treatments for back pain, it very well may be worth your time and effort. In my "former life" working in healthcare, I have seen many people whose lives have been really harmed by narcotic treatment and the devastation that falls onto the rest of the family. It may seem like your wife doesn't care about your pain but I would have to at least wonder if she just sees how bad things got for everyone while you were taking pain medication - and can't feel the pain that you feel (OBVIOUSLY) and to her going back on medication is the worst possible decision. She just doesn't feel the pain that you feel.

Again, I'd really suggest seeking out a good pain clinic and just tell them that at least for now, pain meds are off the table, so what else can they do for you?
